Book excerpt: Excerpt from The Medical Chronicle, 1906: A Monthly Record of the Progress of Medical Science O.s., a Caretaker, aged 57, was admitted into St. George's Hospital on October 3rd, 1906, with all the signs and symptoms of acute general peritonitis. Two days before admission he had taken some castor oil, and soon after felt something go in his inside. There was a history of chronic constipation. Nothing was found to indicate the origin of the trouble, and the abdomen was opened over the appendix, and was found to be filled with a dirty-brown, sour-smelling liquid. The appendix was adherent, but obviously not the cause of the trouble. There were adhesions in the lower part of the abdomen. A sub umbilical incision on the left side was then made, and the gut in the left iliac fossa was found bound down by adhesions, and covered with sloughy lymph. In the pelvis a piece of colon was found which was perforated. Tube and gauze-drain inserted; but patient died 55 hours after operation. Atthe autopsy there was general severe, septic peritonitis. The large bowel on the left side was firmly bound down by old adhesions, and there was much matting in the pelvis. On the antero - external surface of the commencement of the sigmoid there was a small perfora tion with chronically thickened edges, which was found to lead into a stercoral pouch with which the large bowel from the caecum to the rectum was plentifully supplied.
