La idea central del estudio de Horney es que, ante circunstancias adversas, los individuos reaccionan desarrollando determinadas actitudes o estrategias defensivas que les permiten hacer frente al medio y obtener una cierta gratificación. Específicamente, los individuos utilizan tales estrategias para enfrentarse a -o minimizar- los sentimientos de angustia y para conseguir relacionarse con otros. Cuando las citadas estrategias son exageradas o inapropiadas, se denominan tendencias neuróticas. Horney sostiene que tales tendencias no son por naturaleza instintivas, sino que dependen en gran medida de la situación en que vive una persona. El dilema de la incapacidad neurótica de querer y de la angustiosa necesidad de ser querido, así como el papel del amor como alivio o ilusión, entre otras cosas, son estudiados en este libro con detenimiento, penetración y claridad.
