Book excerpt: Despite the lack of literature and inconsistencies in research aims and methods, it is evident that dietary intake has been shifting from traditional food consumption patterns to diets that are heavily reliant on imported and increasingly processed foods in Fiji. Availability of food items varies depending on location, however there is evidence of food environments with high availability of energy dense, nutrient poor foods, in both rural and urban locations. Food environments around vulnerable populations, for example schools, are of concern, with 80 percent of the outlets surveyed within 400 m school zones selling sugar-sweetened beverages. While the process of developing policies that support a healthy diet is enabled by Fiji’s whole-of-government commitment to improving food nutrition security, there are competing priorities and points of incoherence that deter this, particularly relating to economic growth. A range of specific policy measures have been introduced in Fiji by different government sectors that influence all facets of the food system. However, capacities required to develop and deliver policies are limited across government, primarily by the allocation of resources and technical expertise. This reduces the effectiveness of existing policy measures to support healthy diets, and the progression of new measures. A range of gaps and opportunities exist among the policy measures identified, and entry points lie throughout the food system and associated policy environment to improve dietary behaviour in Fiji.

Book excerpt: The percentage of population suffering from overweight or obesity is increasing worldwide, both in developed and developing countries, and for all subpopulations of age and gender (although data on the incidence of those problems seem to differ somewhat across sources; see Díaz-Bonilla and Paz, 2019). High body mass is a risk factor for many non-communicable diseases such as diabetes, high blood pressure, coronary heart disease and stroke. High Body Mass also tends to increase the risks of various types of cancer, gallbladder disease, musculoskeletal disorders and respiratory symptoms among others. In consequence, it constitutes one of the leading risk factors causing early death and disability, as measured in disability-adjusted life years (DALYs).1 It is estimated that in recent years overweight and obesity contributed to about an estimated 4 million deaths (7.1% of all deaths) and 120 million DALYs worldwide (4.9% of all DALYs among adults); overweight and obesity also carry economic costs, estimated globally in US$500 billion per year (Development Initiatives, and Institute for Health Metrics and Evaluation (IHME) , 2018). Consequently, there is an increasing interest to develop policies and interventions that may reduce the incidence of overweight and obesity.

Book excerpt: OBJECTIVES: Cities are important settings for production and prevention of non-communicable diseases. This article proposes a conceptual framework for identification of opportunities to prevent diet-related non-communicable diseases in cities. It compares two cities, Cape Town in South Africa and New York City in the United States, to illustrate municipal, regional, national and global influences in three policy domains that influence NCDs: product formulation, shaping retail environments and institutional food practices, domains in which each city has taken action. STUDY DESIGN: Comparative case study. METHODS: Critical analysis of selected published studies and government and non-governmental reports on food policies and systems in Cape Town and New York City. RESULTS: While Cape Town and New York City differ in governance, history and culture, both have food systems that make unhealthy food more available in low-income than higher income neighborhoods; cope with food environments in which unhealthy food is increasingly ubiquitous; and have political economies dominated by business and financial sectors. New York City has more authority and resources to take on local influences on food environments but neither city has made progress in addressing deeper social determinants of diet-related NCDs including income inequality, child poverty and the disproportionate political influence of wealthy elites. CONCLUSIONS: Through their intimate connections with the daily lives of their residents, municipal governments have the potential to shape environments that promote health. Identifying the specific opportunities to prevent diet-related NCDs in a particular city requires intersectoral and multilevel analyses of the full range of influences on food environments.

This change was accompanied by lifestyle changes and spurred a global nutrition transition. Today the world faces a global syndemic of obesity, undernutrition, and climate change. A new call to action to create food systems that nourish people and sustain the planet is needed. Traditional and indigenous food systems have long been recognized as systems that can both support good human nutrition as well as maintain a balance with nature. There is an underutilized knowledge base around traditional and indigenous food systems. This includes the knowledge of nutritious species, traditional culinary preparations, and cultural practices. Greater agricultural production of underutilized species can result in more sustainable agricultural and food systems which can also help improve livelihoods and food security. Traditional and indigenous cultural practices with respect to both land and water management, as well as culinary practices, contribute to both sustainable food production and consumption. These practices require a greater evidence base in order to be incorporated into public health nutrition initiatives related to improving dietary quality, such as food-based dietary guidelines for example. An increased focus on the importance of local, traditional, and indigenous food systems and nutrition could therefore help countries to improve human nutrition and, ideally, help mitigate the global syndemic of obesity, undernutrition, and climate change. This Research Topic will focus on documenting diverse local food systems and promoting elements within them that can help improve nutrition and health – both human and planetary - in various ways including the livelihood development of knowledge holders.
