Book excerpt: This white paper aims to provide Northern and Southern Development partners and decision-makers with a better understanding of a) why spillover of disease from wildlife to humans occurs, and why these zoonotic disease outbreaks can spread and become epidemics and pandemics such as COVID-19 and b) what they can do to prevent, detect and respond to future spillover events, with a special focus on priority interventions at the human-wildlife-livestock interfaces. It has been produced as part of the Sustainable Wildlife Management (SWM) Programme, which will deliver critical lessons on how to prevent, detect and respond to future spillover events with appropriate national and transboundary policies and practices in the context of the SWM partner sites. The SWM Programme is a major international initiative to improve the conservation and sustainable use of wildlife in forest, savannah and wetland ecosystems. Field projects are being implemented in 13 African, Caribbean and Pacific countries. The aim is to: improve how wildlife hunting is regulated; increase the supply of sustainably produced meat products and farmed fish; strengthen the management capacities of indigenous and rural communities; and reduce demand for wild meat, particularly in towns and cities. Although we consider conservation medicine an emerging field, the concept is the result of the long evolution of transdisciplinary thinking within the health and ecological sciences and the better understanding of the complexity within these various fields of knowledge. Conservation medicine was born from the cross fertilization of ideas generated by this new transdisciplinary design. It examines the links among changes in climate, habitat quality, and land use; emergence and re-emergence of infectious agents, parasites and environmental contaminants; and maintenance of biodiversity and ecosystem functions as they sustain the health of plant and animal communities including humans. During the past ten years, new tools and institutional initiatives for assessing and monitoring ecological health concerns have emerged: landscape epidemiology, disease ecological modeling and web-based analytics. New types of integrated ecological health assessment are being deployed; these efforts incorporate environmental indicator studies with specific biomedical diagnostic tools. Other innovations include the development of non-invasive physiological and behavioral monitoring techniques; the adaptation of modern molecular biological and biomedical techniques; the design of population level disease monitoring strategies; the creation of ecosystem-based health and sentinel species surveillance approaches; and the adaptation of health monitoring systems for appropriate developing country situations. New Directions of Conservation Medicine: Applied Cases of Ecological Health addresses these issues with relevant case studies and detailed applied examples. New Directions of Conservation Medicine challenges the notion that human health is an isolated concern removed from the bounds of ecology and species interactions. The 2018 TZG updates and expands on the guidance in the one previous jointly-developed, zoonoses-specific guidance document: the 2008 Tripartite “Zoonotic Diseases: A Guide to Establishing Collaboration between Animal and Human Health Sectors at the Country Level”, developed in WHO South-East Asia Region and Western Pacific Region. The 2018 TZG supports building by countries of the resilience and capacity to address emerging and endemic zoonotic diseases such as avian influenza, rabies, Ebola, and Rift Valley fever, as well as food-borne diseases and antimicrobial resistance, and to minimize their impacts on health, livelihoods, and economies. It additionally supports country efforts to implement WHO International Health Regulations (2005) and OIE international standards, to address gaps identified through external and internal health system evaluations, and to achieve targets of the Sustainable Development Goals. The 2018 TZG provides relevant country ministries and agencies with lessons learned and good practices identified from country-level experiences in taking OH approaches for preparedness, prevention, detection and response to zoonotic disease threats, and provides guidance on multisectoral communication, coordination, and collaboration. It informs on regional and country-level OH activities and relevant unisectoral and multisectoral tools available for countries to use.
