Book excerpt: Since the late 1990s, the United States has funneled billions of dollars in aid to Colombia, ostensibly to combat the illicit drug trade and State Department-designated terrorist groups. The result has been a spiral of violence that continues to take lives and destabilize Colombian society. This book asks an obvious question: are the official reasons given for the wars on drugs and terror in Colombia plausible, or are there other, deeper factors at work? Scholars Villar and Cottle suggest that the answers lie in a close examination of the cocaine trade, particularly its class dimensions. Their analysis reveals that this trade has fueled extensive economic growth and led to the development of a "narco-state" under the control of a "narco-bourgeoisie" which is not interested in eradicating cocaine but in gaining a monopoly over its production. Latvia has successfully consolidated its hospital sector and strengthened primary care. Average length of stay in hospital fell by almost 15% between 2005 and 2013, and GPs are now required to follow up on patients who called for emergency medical assistance but were not hospitalised. OECD health systems could learn much from these reforms as well as longer-standing institutions, such as Latvia's feldshers (physician assistants). Latvia nevertheless faces important challenges to improve the performance of its health system. Up to one in five Latvians report forgoing health care because of the cost; waiting times for key diagnostic and treatment services can be long; and inclusion of key treatments in the publicly-funded benefits basket does not always reflect latest best practice. Critically, the health system lags behind many OECD countries in the extent to which data are used to systematically measure, compare and improve the performance of services, especially at more granular provider or local levels. The salt trade consistently accounted for roughly 10 percent of government income. In the town of La Salina de Chita, in Boyaca province, thermal springs offered vast amounts of salt, and its procurement and distribution was placed under the jurisdiction of the Ministry of Finance. Focusing his study on La Salina, Joshua M. Rosenthal presents a fascinating glimpse into the workings of the early Colombian state, its institutions, and their interactions with local citizens during this formative period. Although historians have cited the state's weakness and, in many cases, its absence in local affairs, Rosenthal counters these assumptions by documenting the primary role the state held in administering contracts, inspections, land rights, labor, and trade in La Salina, contending that this was not an isolated incident.
