Available in PDF, EPUB and Kindle. Book excerpt: The term "patient safety" rose to popularity in the late nineties, as the medical community -- in particular, physicians working in nonmedical and administrative capacities -- sought to raise awareness of the tens of thousands of deaths in the US attributed to medical errors each year. But what was causing these medical errors? And what made these accidents to rise to epidemic levels, seemingly overnight? Still Not Safe is the story of the rise of the patient-safety movement -- and how an "epidemic" of medical errors was derived from a reality that didn't support such a characterization. Physician Robert Wears and organizational theorist Kathleen Sutcliffe trace the origins of patient safety to the emergence of market trends that challenged the place of doctors in the larger medical ecosystem: the rise in medical litigation and physicians' aversion to risk; institutional changes in the organization and control of healthcare; and a bureaucratic movement to "rationalize" medical practice -- to make a hospital run like a factory. If these social factors challenged the place of practitioners, then the patient-safety movement provided a means for readjustment. In spite of relatively constant rates of medical errors in the preceding decades, the "epidemic" was announced in 1999 with the publication of the Institute of Medicine report To Err Is Human; the reforms that followed came to be dominated by the very professions it set out to reform. Weaving together narratives from medicine, psychology, philosophy, and human performance, Still Not Safe offers a counterpoint to the presiding, doctor-centric narrative of contemporary American medicine. It is certain to raise difficult, important questions around the state of our healthcare system -- and provide an opening note for other challenging conversations.

Available in PDF, EPUB and Kindle. Book excerpt: In C.S. Lewis's classic The Lion, The Witch and the Wardrobe, Lucy asks if Aslan the lion is safe. It is quickly clarified that Aslan is not safe...but he is good. That concept serves as the foundation for this collection of short stories. While written from a Christian worldview, our goal isn't comfort food for Christians or G-rated stories that offer simplistic lessons. Instead, we're serving up stories sharpened by faith. Stories that will engage, challenge, entertain, and stretch the reader. These stories aren't necessarily safe...but without question, they are good. The stories in this book--from such outstanding Chrstian writers as Jerry Jenkins, Michael Morris, Sally John, and the editor Bret Lott--are by no means safe. Like the parables of Christ, they surprise, unsettle, and even shock. They depict doubt, loss, abandonment, failure, and betrayal as well as elation and triumph. But they also deeply and meaningfully explore the human condition in relation to a God who loves us and brings us joy and hope.

Book excerpt: Even if you haven't been hurt by domestic violence, someone you know has and wishes they could tell you about it. Perhaps you are a therapist, teacher, academic, or social worker who wants to help those who are suffering. Or maybe you are in an abusive relationship and need to know that you are not alone. The poems, memoirs, and creative nonfiction pieces collected here tell of real incidents of abuse, as well as of those who left destructive and unsalvageable relationships. The beauty and truth of the language, as well as the honesty and courage, set this anthology apart from self-help manuals and academic treatises on domestic violence. This book offers a path forward to healing, health and fulfillment, using the power of art to give voice where voice has been stifled, forgotten, overlooked or denied.

Book excerpt: In this adaptation of his bestselling book, All In, now distilled in this essential edition, Mark Batterson shows us there is nothing more thrilling in life than allowing Jesus to make us dangerous. In this compelling read, Mark Batterson challenges our mistaken beliefs: That God wants to send us to safe places to do easy things That faithfulness is holding the fort That playing it safe is safe That there is any greater privilege than sacrifice And that radical is anything but normal Instead, Batterson maintains that Jesus did not die to keep us safe; he died to make us dangerous. Here is a challenge to go all in and all out by fully surrendering your life to daring plan God has for you. The message of Not Safe is simple: If Jesus is not Lord of all then Jesus is not lord at all. It's all or nothing. Now or never. Not safe or safe. Spanish edition also available.

Such ways of thinking are typically greater expressions of society’s thinking (whether in the church or in the world) than what Jesus taught. Jesus is Not Safe concerns some such ways, ways that a thoughtful disciple of Jesus needs to challenge.

Available in PDF, EPUB and Kindle. Book excerpt: Based on an EU workshop at the end of 2005, the book discusses risk and our food supply. The introductory chapter will discuss all aspects of risk and how it applies to food, from risk classification to risk management. Following a discussion of risk, the authors will present three different case studies that will emphasize the following issues: • What do we want as individuals, as a society • What is the political context of the risk discussion • When do we act and what are the costs of not acting/acting • International trade and legal issues • Moral dimensions of decision making • How do we deal with the disproportionate "power" of the various stakeholders • Rationality/emotive aspects of argumentation (connection between perception /live experiences, knowledge) • What are facts- and do they change with time • Psychological aspects: rapture of trust; the need for certainty; connection between danger, fear and risk
