It can lead to severe complications and increases the risk of heart disease, stroke, and death. Blood pressure is the force exerted by the blood against the walls of the blood vessels. This book is a concise guide to the definition, diagnosis and management of high blood pressure, for practising and trainee cardiologists. Beginning with an introduction to defining and measuring blood pressure, the next chapter examines clinical features and investigation, and lifestyle management. The following sections discuss different classes of antihypertensive drugs covering action mechanisms, pharmaceutical characteristics, side effects, and contraindications. A complete chapter is dedicated to newer therapies for systemic hypertension, including renal denervation therapy and baroreceptor activation therapy. The next part of the book analyses management of hypertension with a variety of comorbid conditions such as obesity, diabetes, heart failure, and chronic kidney disease. The text concludes with an overview of current WHO (World Health Organisation), ESC (European Society of Cardiology), ACC (American College of Cardiology), and Indian recommendations and guidelines for the detection, evaluation and management of hypertension. This practical manual is highly illustrated with images, tables and diagrams to further assist learning.

Book excerpt: It is well known that cardiovascular events occur more frequently in the morning as blood pressure (BP) levels have been shown to increase during the period from night to early morning. In recent years, clinical research using ambulatory blood pressure monitoring (ABPM) or home BP monitoring has clarified that morning BP and BP surge are more closely related to the cardiovascular risk than clinical BP. This practical manual from field leading expert, Dr. Kazuomi Kario, reviews recent evidence on morning and nocturnal hypertension and the IT technologies physicians can use to support patients in home monitoring BP. Guidance on management via antihypertensive drugs is also discussed and with the aim of promoting perfect 24 hour BP control.

Book excerpt: ESSENTIAL MANUAL OF 24-HOUR BLOOD PRESSURE MANAGEMENT Hypertension is one of the greatest threats to human health. The World Health Organization (WHO) estimates that 1.13 billion people worldwide have hypertension. In 2017, new guidelines for managing hypertension were published by the American Hypertension Association (AHA), guidelines which lowered the diagnosis thresholds of hypertension, and thereby increased the prevalence of hypertension. As such, hypertension is now recognized as a more serious and widespread a condition than ever before. In this new edition of the Essential Manual of 24-Hour Blood Pressure Management, the author emphasizes that lowering the blood pressure (BP) and restoring the BP profile with adequate circadian rhythm is essential for a long life without cardiovascular events. The author also introduces updated evidence for managing hypertension throughout 24-hour periods, from morning to nocturnal hypertension. The Essential Manual of 24-Hour Blood Pressure Management, Second Edition, will be an essential companion for doctors who wish to provide evidence-based medicine and be familiar with the most cutting edge technology on monitoring BP. Medical researchers and students will also value the author’s many insights, drawn from his distinguished career.

Book excerpt: The prevention and correction of high blood pressure obviously is not an end in itself but a means of preventing cardiovascular complications. It is tempting to res ort to antihypertensive drugs, for the simple reason that current preparations are easy to dose and to take. There is a variety of reasons, however, to take advantage of the antihypertensive potential of nonpharmaco logical approaches first. For one thing, the use of tablets tends to accentuate the negative effects oflabelling a subject as hypertensive. The act oftaking tablets per se may elicit a spectrum of subjective adverse effects, as we have learned from placebo-controlled therapeutic trials. Even more importantly, the benefits of antihypertensive drug treatment are largely confined to the reduction of stroke and disease of the large vessels. With regard to incidence of cardiac events, some modest beneficial effects have been registered, such as a decrease in congestive heart failure and in the fatality rate of myocardial infarction. The effects on the incidence of coronary heart disease are only marginal at best. This disappointing finding has been ascribed to various factors: unwanted metabolie effects of anti hypertensive drugs, inadvertent drops in diastolic pressure interfering with coro nary perfusion, and, last but not least, failure to eliminate risk factors other than high blood pressure (smoking, alcohol abuse, lipid and glucose disturbanees, obesity, and the hemodynamic disadvantages of a sedentary life). Most of these additional noxious factors are refractory to drug treatment and call for modifica tion of life-style.
