Evidence indicates that hospital-acquired infections pose an immense hazard for patients within the hospital environment, and healthcare workers, including nurses are involved in the transmission of these infections. This explains why health organizations such as WHO, The Joint Commission and Centers for Disease Control have embarked on spearheading education and awareness on hand hygiene. Despite these efforts, evidence indicates that healthcare workers continue to exhibit low levels of hand hygiene compliance. Therefore, this essay will demonstrate how nurses can assume a leadership role within the hospital settings in accordance with the health safety standards (National Safety and Quality Health Services) as outlined by the Australian Commission on Safety and Quality in Health Care [ACSQHC]. It will be discussed how nurses can adopt hand hygiene strategies to improve compliance to the national hand hygiene initiative in the wards as one of the key approaches for preventing and controlling hospital-acquired infections.

In the United States alone, nosocomial infections kill a staggering number of patients and sicken countless others (Smith, Lokhurst, 2009). Naturally, this generates a great deal of concern in many health care settings and is a primary concern to those in the medical community, especially since the safety of patients is being compromised. While not all hospital-acquired infections can be prevented, the vast majority can. The startling reality is that studies have proven most of the infections acquired in the hospital setting are transmitted from patient to patient via the hands of healthcare workers (Smith, Lokhurst, 2009). Hand sanitization, has long been deemed a first-line of protection in the prevention of these infections. Of equal significance, has been the struggle for compliance with hand hygiene procedures that are vital to the safety of patients in the hospital. According to the results of a European hospital study, nurses' practice with hand hygiene is weak, and went on to reiterate, the compliance level of the staff is unacceptable (Malliarou, Sarafis, Zyga, Constantinidis, 2013). As evidence suggests, there is an urgent need to undertake robust methodologies in order to effectively increase hygiene compliance to safeguard patients. This written proposal presents the following; in the emergency department (ED), will the use of visual cues, in comparison to only standard education, actually increase hand hygiene compliance for the ED staff, within 6 months? There are many reasons why staff fail to clean their hands consistently and one identified barrier happens to be ineffective or insufficient education (Joint Commission, 2010). Standard education has been utilized in many hospitals and yet, it is well published that hand hygiene compliance rates remain inadequate. It is proposed that visual cues are needed to encourage health care workers to disinfect their hands in an effort to increase hand hygiene compliance. According to a 2014 study, the absence or presence of reminders of hand washing in public restrooms and a university campus were studied to determine if hand washing compliance was affected, and the results were a resounding, yes! The results confirmed, creating obvious environments and layouts that provide visual cues and convenience for workers has been associated with increased hand washing rates (Ford, Boyer, Menachemi, Huerta, 2014). In order to employ this proposal, hospital leadership teams will be provided with an implementation plan consisting of a step by step proposition of recommended interventions such as displaying visual cues at the points of patient care. Ongoing evaluations will be conducted throughout each phase to ascertain effectiveness and identify areas of improvement. Plans will be disseminated to identified stakeholders to encourage support and ongoing involvement.

Abstract: Introduction: Hand hygiene, the most effective and economical way to prevent the transmission of many bacteria and viruses in healthcare settings, yet, its compliance rate remained unacceptably poor in many countries. Compliance with hand hygiene among healthcare workers increased substantially during outbreak of serious acute respiratory symptoms (SARS) in 2003 but decreased to the baseline after SARS, to improve compliance rate sustainable, many hand hygiene intervention programs have been established since 2003. However, the effectiveness of these programs were in big variations. The objectives of this review were to identify the effectiveness of these hand hygiene intervention programs. Methods: This review was conducted using the search engines - PubMed. The key words "hand hygiene," "hand-washing," "compliance," "healthcare personnel," and "program*" were used for searching literature published from 2003 to July 2012.. Forty-Seven articles were initially obtained. After screening and reviewing of these articles,9 articles met the requirements for inclusion and exclusion criteria were included in this review. The outcome measure was set as the differences and relative ratios in the observed hand hygiene compliance rate before and after intervention. Results: Results showed that 8 hand hygiene programs were effective while 1 programme was ineffective in improving hand hygiene compliance rates. Of the eight studies that reported improvement in compliance with hand hygiene, improvement in compliance rate after the interventions was ranged from (24% to 50%) while the relative ratio of compliance (after vs. before intervention) was ranged from 1.78 to 4. The study that reported no improvement in compliance with hand hygiene had a relative high naseline hand hygiene compliance rate of 61% Conclusion: Hand hygiene programs that improve compliance rate among healthcare workers were effective overall. The effectiveness of hand hygiene programs seemed to be positively associated with the numbers of intervention types involved. Proper hand washing and the understanding of this responsibility can improve patient care. The Joint Commission reported an overall 36% compliance in hand hygiene over the last three decades. The issue that every nurse is aware of the fact that hand washing it the most important intervention, but continues to dismiss this practice time after time. Evidence Based Framework: The formation of the project has been based on the Steven's Ace Star Model. Through this framework, the problem has been identified, explored and an intervention has been chosen to create improvements through evidence based practice (EBP). Nightingale's Environmental theory has been connected to this project to support the need for proper hand hygiene. Methods: Effort was made to increase compliance in hand hygiene, and implement evidence-based education to improve hand hygiene among nurses and other health care workers. With this effort, there has been a proposed plan for sustainability by continuing to employ frequent hand hygiene trainings at the health care facility. Findings: Findings reported from Infection Control reported an overall increase in awareness of proper hand hygiene. By using participants from the in service to monitor hand hygiene among nurses, new understand of proper hand hygiene was reported. More through grading was done and an increase in awareness of troubled areas that need to be targeted in all units found. Conclusion: Following the project, the conclusion can be drawn that awareness was increased from the intervention. By educating a small sample of nurse, word was spread and continued to filter throughout the facility. Other departments were made more aware of special areas such as hand hygiene pre and post patient contact that they had been suffering in and were not fully aware was included in hand hygiene.
