Book excerpt: In recent years, there has been a wealth of new information on the physiological and biochemical consequences of hypoxia, or low blood levels of oxygen. This new volume discusses the implications of these new findings on the pathophysiology, development, and treatment of hypoxic metabolic acidosis. The volume is part of the Clinical Physiology series sponsored by the American Physiological Society, and is based on a FASEB symposium held in May 1988. Hypoxia was once thought to affect organs in a similar manner, but it is now known that each is affected differently. The author shows how hypoxia and metabolic acidosis affect the heart, lungs, blood vessels and other organs at the cellular level, the tissue level, and finally, at the level of the entire organ. The book then proceeds to a description of the situations in which hypoxic metabolic acidosis develops, such as during high altitude exposure, cardiac arrest, and lactic acidosis. The last few chapters give an overview of treatment. Traditional therapy has consisted largely of the intravenous administration of sodium bicarbonate. This method has come under increasing scrutiny however, and the range of problems associated with the use of sodium bicarbonate is examined fully. Newer alternative agents for managing hypoxic acidosis are reviewed as well. This up-to-date review of hypoxia and metabolic acidosis should be of interest to physiologists, internists, cardiologists, chest physicians, anesthesiologists, and intensive care specialists.

This spirit of history set the tone of the conference and in the opening lecture the contribution that this part of the world has given to the understanding of oxygen transport to tissue was highlighted. In particular, the contribution of Galen of Pergamon (129-200) was discussed who for the first time demon strated that blood flowed through the arteries and whose view on the physiology of the circulation dominated the ancient world for well over a millennium. A forgotten chapter in the history of the circulation of the blood is the contribution made by Ibn al Nafis of Damascus (1210-1280) who for the first time described the importance of the pulmonary circulation by stating that all venous blood entering the right ventricle ofthe heart passes to the left ventricle, not through pores in the septum of the heart as had been postulated by Galen, but through the circulation of the lungs.

It was attempted to provide criteria for the distinction between the primary and secondary phenomena on a morpho logical and functional level, on the basis of the kinetics involved and, most importantly, regarding the different specific manifestations, such as disturbances of microcirculation, aspects of the blood-brain barrier, and of cellular structure and function at a molecular level. Although it was not expected that a grand unifying hypothesis will be reached recon cilable with the many, occasionally opposing views on such a complex subject, nevertheless, the present volume attains an appropriate result. It can best be described as a mosaic of many different pieces which only as an ensemble reflect the current state of the art.
