Book excerpt: The research and outcomes presented in this book gather evidence concerning both the pathogenesis and treatment of functional dyspepsia. It provides the latest information on this common non-organic disease, indicating its characteristic pathogenesis based on the brain-gut interaction and micro-environment and evidence gleaned from clinical treatment. Since the pathogenesis is associated with psychology, neurology, endocrinology and bacteriology in addition to gastroenterological physiology, it is often intractable and finding a suitable treatment rationale is challenging. Furthermore, the pathogenesis varies around the world and the efficacy of treatment using standard drugs varies among different populations worldwide; accordingly, this book highlights evidence gained in clinical trials in Japan. Functional Dyspepsia is a milestone produced by respected experts. Addressing unique topics and new findings of treatment including challenging and/or future rationales, it offers an invaluable resource for general clinicians, gastroenterologists, and basic researchers alike.

Available in PDF, EPUB and Kindle. Book excerpt: Although dyspepsia has been investigated for a long period of time, there is no international agreement on what constitutes this condition nor any standardized guidelines. National guidelines followed by practitioners in different countries vary in diagnostic and therapeutic approach, underlining the necessity for a unique definition worldwide. Dyspepsia in Clinical Practice summarizes the current guidelines while offering a unified, practical definition of dyspepsia, and a diagnostic algorithm with an emphasis on the upper gastrointestinal endoscopy and rational first-line therapeutic approach based on epidemiology, pathophysiology, clinical presentation, diagnostic workup and response to previous therapy. Up-to-date scientific information about dyspepsia is presented from a practical, clinician's point of view. Written by experts in the field, this volume addresses dyspepsia in childhood and in the elderly, a very important issue often insufficiently emphasized in the literature. Guidelines are provided that can be easily followed in clinical practice, leading to a reduction in costs and increased patient safety. Dyspepsia in Clinical Practice will be of great value to gastroenterologists, internists, primary care physicians, pediatricians, infectious disease specialists, residents and fellows in training.

These disorders are extremely common. They are characterized by symptoms and the lack of structural lesions that can be identified with clinically available routine diagnostic tests. Several functional abnormalities are now believed to play a role in the development of the symptoms. This book summarizes the presentations at the International Falk Symposium No. 99 `Functional Dyspepsia and Irritable Bowel Syndrome: Concepts and Controversies' that took place on May 27-28, 1997. This symposium brought together clinical and basic researchers and clinicians to improve interdisciplinary communication. The meeting comprehensively covered basic and clinical aspects of these disorders, and it not only summarized the current knowledge, but also identified scientific questions that need to be addressed in the future.

Available in PDF, EPUB and Kindle. Book excerpt: Dyspepsia is a disorder which affects a large portion of the population. However, it is very important to differentiate between commonly presenting symptoms like gastroesophageal reflux disease, functional dyspepsia, and irritable bowel syndrome. Also various classifications and treatment protocols have been developed and revised to provide a rational platform for the clinicians. The ROME III classification has to a large extent given a direction to the clinicians to base their practice on, however, is a classification based on clinical trials and is really helpful in daily practice. In this issue we have tried to focus on the need for classification and approach to the patient with dyspepsia.

Book excerpt: Gastroscopy is the primary investigation for dyspeptic patients who are aged more than 55 years or have alarming symptoms or use non-steroidal anti-inflammatory drugs (NSAIDs). The investigation of dyspepsia in patients aged less than 55 years should start by establishing the presence of Helicobacter pylori infection. Helicobacter-positive patients are treated with triple therapy . Before further investigations are carried out, a 2–4 week therapeutic trial with a proton pump inhibitor (PPI) is indicated in Helicobacter-negative patients aged less than 55 years whose symptoms are not suggestive of an organic disease and who do not use NSAIDs . Upper abdominal ultrasonography is not useful as a primary investigation.Don’t forget the possibility of an ischaemic heart disease in a patient at risk.
