Book excerpt: The proposed abolition of Primary Care Trusts and transfer of their commissioning functions to GP consortia have been greeted with intense excitement by some GPs, and with extreme trepidation by others. This book explains what commissioning is and how to do it, enabling clinicians to flourish as commissioners in the new NHS. Some topics that will be familiar to readers, such as long term conditions management, patient safety and the wider determinants of health, are explored here from a commissioning perspective. Others which may be less familiar, such as how to delegate, how to improve productivity, management theories and the content of financial accounts, are explored in detail. Additional skills needed by commissioning GPs - fostering innovation, assertiveness and emotional competence as well as negotiation, presentation and even media management skills - are outlined in a clear and straightforward fashion, while the necessary attitudes involved are described with reference to the Nolan Principles of Public Life. A straightforward yet comprehensive guide, Effective GP Commissioning - Essential Knowledge, Skills and Attitudes will be essential for GPs, prospective GPs, and those with an interest in effective commissioning. 'A useful resource for all GPs whatever their level of engagement and indeed for non-clinicians also. Sunil Gupta is clearly a GP and business magpie who has an incredible ability to gather, organise and present useful information. The content of this book is well-researched linking considerable volumes of information in a very useful, constructive manner.' - From the Foreword by Simon Gregory

Book excerpt: General Practice has never been an easy occupation. In the past there were some clear structures and routes for new GPs to take. You could expect to join a practice and progress through the ranks, growing in seniority and confidence. You might have been informally mentored by one of the older colleagues; learning management and business skills gradually. It is no longer like this. The market has changed, the range of options is wider, and the way through the labyrinth is far from clear. You might find yourself confused and overwhelmed by all the new opportunities, possibilities and prospects. This book helps to clarify uncertainties and guide you towards the right path. It offers ways to formulate strategies when planning your career and maps out the landscape of general practice, enabling you to make, confident and informed decisions about your professional future. Supporting the initiative of First5(R), The New GP's Handbook is highly recommended for newly qualified GPs who will find it answers so many of their questions and helps make the first five years (and beyond) in general practice more understandable, productive and enjoyable. Experienced GPs too, will find the guide invaluable as a current, general overview. General practice is an exciting and rewarding career which provides a host of opportunities for new GPs entering the profession today. Finding the right job, achieving a good work-life balance and developing a culture of lifelong learning are vital not only for the fulfilment of new GPs themselves but for the future of the profession. This book will help you achieve these objectives. Clare J Taylor, in the Foreword

In one way or another, part of this important task will fall to physicians. How do they make judgments about resource stewardship, and how should they do so? How can they make such decisions in a manner that is compatible with their clinical duties to patients? In this book, philosophers, bioethicists, physicians, lawyers and health policy experts make the case that priority setting and rationing contribute significantly to the possibility of affordable and fair healthcare and that clinicians play an indispensable role in that process. The book depicts the results of a survey of European physicians about their experiences with rationing and other cost containment strategies, and their perception of scarcity and fairness in their health care systems. Responding to and complementing these findings, commentators discuss why resource allocation and bedside rationing is necessary and justifiable. The book explores how bedside rationing relates to clinical judgments about medical necessity and medical indications, marginal benefits, weak evidence based medicine, off-label use. The book highlights how comparative studies of health care systems can advance more effective and fair bedside rationing through learning from one another. From a practical standpoint, the book offers a number of strategies for health care systems and clinicians to work in tandem to allocate and ration resources as fairly as possible: how to foster more attention to fairness when rationing at the bedside, how to avoid exacerbating health disparities when allocating resources, how to teach about bedside rationing to students, how to discuss rationing more explicitly in the public arena and in the doctor's office.

Available in PDF, EPUB and Kindle. Book excerpt: Epistemic injustice was conceptualized by Fricker as a form of social injustice, which occurs when people’s authority ‘as a knower’ is ignored, dismissed, or marginalized. It is attracting increasing interest in the mental health field because of the asymmetries of power between people using mental health services and mental health professionals. People experiencing mental health distress are particularly vulnerable to epistemic injustice as a consequence of deeply embedded social stigma, negative stereotyping, and assumed irrationality. This is amplified by other forms of stereotyping or structural discrimination, including racism, misogyny, and homophobia. Consequently, individual testimonies may be discounted as both irrational and unreliable. Epistemic injustice also operates systemically reflecting social and demographic characteristics, such a race, gender, sexuality or disability, or age.
