Book excerpt: Continuous Quality Improvement (CQI) methods are increasingly widely used to bridge the gaps between the evidence base for best clinical practice, what actually happens in practice, and the achievement of better population health outcomes. Among a range of quality improvement strategies, CQI methods are characterised by iterative and ongoing use of specific processes to identify quality problems, develop solutions, and implement and evaluate changes. The application of CQI processes in health is evolving and evidence of their success continues to emerge. There is, however, a need to enhance understanding of how best to implement, scale-up and evaluate CQI programs for the purpose of improving quality of care and population health outcomes in different contexts. This research topic aims to attract articles that add to knowledge of useful approaches to tailoring CQI methods for different contexts or purposes, and for implementation, scale-up and evaluation of CQI interventions/programs.

This book was released on 2008 with total page pages. Available in PDF, EPUB and Kindle. Book excerpt: School districts and administrators across the country are struggling to meet the mandates of the No Child Left Behind (NCLB) Education Act. The mandates and expectations of meeting adequate yearly progress as spelled out by NCLB were clear and the pressure was placed squarely on the shoulders of administrators and educators across the nation. This study was an implementation evaluation of the Continuous Quality Improvement (CQI) process, within three schools within a large urban school district, in southern Missouri. The goal of the study was to measure what, if any, change occurred in the perceptions of teachers within the buildings that had implemented CQI as compared to the year prior to CQI implementation and as compared to similar buildings that had not implemented CQI. The study concentrated on teachers' perceptions of the seven characteristics of CQI: Leadership, Strategic Planning, Student/Stakeholder Focus, Information and Analysis, Faculty/Staff Focus, Process Management, and Results; while utilizing data from the Missouri School Improvement Staff Survey. An implementation evaluation was chosen due to the school district being very early in its stages of CQI deployment and to ascertain the extent which implementation has changed teacher perceptions.

Book excerpt: Second in a series of publications from the Institute of Medicine's Quality of Health Care in America project Today's health care providers have more research findings and more technology available to them than ever before. Yet recent reports have raised serious doubts about the quality of health care in America. Crossing the Quality Chasm makes an urgent call for fundamental change to close the quality gap. This book recommends a sweeping redesign of the American health care system and provides overarching principles for specific direction for policymakers, health care leaders, clinicians, regulators, purchasers, and others. In this comprehensive volume the committee offers: A set of performance expectations for the 21st century health care system. A set of 10 new rules to guide patient-clinician relationships. A suggested organizing framework to better align the incentives inherent in payment and accountability with improvements in quality. Key steps to promote evidence-based practice and strengthen clinical information systems. Analyzing health care organizations as complex systems, Crossing the Quality Chasm also documents the causes of the quality gap, identifies current practices that impede quality care, and explores how systems approaches can be used to implement change.

Book excerpt: Cardiac arrest can strike a seemingly healthy individual of any age, race, ethnicity, or gender at any time in any location, often without warning. Cardiac arrest is the third leading cause of death in the United States, following cancer and heart disease. Four out of five cardiac arrests occur in the home, and more than 90 percent of individuals with cardiac arrest die before reaching the hospital. First and foremost, cardiac arrest treatment is a community issue - local resources and personnel must provide appropriate, high-quality care to save the life of a community member. Time between onset of arrest and provision of care is fundamental, and shortening this time is one of the best ways to reduce the risk of death and disability from cardiac arrest. Specific actions can be implemented now to decrease this time, and recent advances in science could lead to new discoveries in the causes of, and treatments for, cardiac arrest. However, specific barriers must first be addressed. Strategies to Improve Cardiac Arrest Survival examines the complete system of response to cardiac arrest in the United States and identifies opportunities within existing and new treatments, strategies, and research that promise to improve the survival and recovery of patients. The recommendations of Strategies to Improve Cardiac Arrest Survival provide high-priority actions to advance the field as a whole. This report will help citizens, government agencies, and private industry to improve health outcomes from sudden cardiac arrest across the United States.
